CAHS Athletic Clearance Process
No student-athlete may participate in tryouts, practices, or competitions until they are fully cleared and have received official confirmation!
To be cleared, all student-athletes must:
- Complete the athletic clearance process online through HomeCampus.
- Upload a completed Athletic Clearance Form to their HomeCampus account.
- Have the application reviewed and approved by the Athletic Department.
BEFORE YOU START:
- Plan ahead!
- Submit your clearance at least 1 month before tryouts.
- Sport clearances expire at the end of each school year.
- An updated Pre-Participation Physical Evaluation (PPE) is required each year:

- Must be performed by an approved healthcare provider (MD, DO, NP, or PA only) licensed in the state of California.
- Physicals are only valid for 365 days.
- If your physical expires during your season, you immediately become ineligible until an updated physical is uploaded to HomeCampus.
- All documents must be uploaded to HomeCampus; NO PAPER COPIES ACCEPTED AT THE FRONT DESK OR AT TRYOUTS!
- Each student-athlete must have their own athletic clearance application.

- Parents/guardians are responsible for completing and maintaining updated records.
- Both the student-athlete AND the parent/guardian are required to acknowledge multiple documents on the “Signatures” section.

Step-By-Step Instructions:

1. Go to HomeCampus and click “Create an Account.” 2. Verify your email and log in. 3. Click “Start Clearance Here.” 4. Select the school, and the correct academic year. 5. Select your sport(s). If the student-athlete intends to participate in multiple sports, click “Add New Sport.” 6. Complete all required information sections (insurance information, emergency contacts, medical information). 7. Electronically sign all required documents. 8. Download and print the Athletic Clearance Form. 9. Take the form to your pre-participation/physical exam (PPE) to be completed by an approved healthcare provider. 10. Upload the completed form as a .pdf in HomeCampus. 11. Wait for an email confirmation that your application was approved. 12. If you have multiple student-athletes, repeat this process (beginning at step 3) for each one. |

Review and update all information sections (insurance information, emergency contacts, medical history). 6. Electronically sign all required documents. 7. Make sure your physical exam is up-to-date (If needed: Download and print the Athletic Clearance Form and get an updated physical exam). 8. Make sure a valid physical exam form is uploaded as a .pdf in HomeCampus. 9. Wait for an email confirmation that your application was approved. 10. If you have multiple student-athletes, repeat this process (beginning at step 2) for each one. |

Frequently Asked Questions (FAQs):
When should I start the athletic clearance process?
At least 1 month before tryouts. Earlier is strongly recommended.
When do athletic clearance applications open?
Applications for the 2026-27 school year will open on June 1, 2026.
Do both the student and parent need accounts?
No. A parent creates an account and submits an athletic clearance application for their student-athlete.
I have multiple students participating in athletics, do they each need a separate athletic clearance application?
Yes. See instructions above.
My student-athlete participated in athletics last year, do I need to do everything again?
Yes. You must:

- Select a clearance application for the correct year.
- Update your information.
- Re-sign all required forms.
- Upload a current physical.
I’m playing multiple sports throughout the year—do I need multiple clearances?
No. Each student–athlete only needs to complete one athletic clearance application for the year, but you must add each sport in HomeCampus. See instructions above.
I forgot to add one of my sports at the beginning of this process, is it too late to add more?
No, instead of starting a new clearance application, log in and edit your existing application by clicking “Change Sports” and selecting any additional sport/s your student-athlete wishes to participate in. Make sure to save your changes!
Do I need a new physical every year?
Yes. Physicals are valid for 365 days, and must not expire during your sports season.
What type of healthcare provider can complete the physical?
Per Escondido Union High School District Policy: Physical exams must be performed by an approved healthcare provider licensed in the state of California. Approved healthcare providers:

- MD (Medical Doctor)
- DO (Doctor of Osteopathic Medicine)
- NP (Nurse Practitioner)
- PA (Physician Assistant)
- EUHSD does NOT accept physicals signed by chiropractors (DC).
Our healthcare provider wants to complete the exam on their own forms, is this ok?
Yes as long as the form contains 1) the name of the student-athlete, 2) the date the physical exam was performed, 3) the name and credentials of the physician who performed the exam (preferably with an office stamp), 4) indication that they have been medically evaluated for athletic participation, and 5) have been medically cleared for sport participation either with limitations, or without any restrictions.
What happens if my physical expires mid-season?
You will become ineligible immediately until a new physical is uploaded and approved.
What does “Ready for Review” mean?
It means your application was submitted to the department for review, but you are NOT CLEARED yet. You must wait for an email confirmation that your application was approved.
Can I turn in paper forms instead of uploading them?
No. All documents must be uploaded to HomeCampus. See instructions above.
How long do I have to wait for my application to be reviewed and approved?
Please allow 3 business days for the athletics department to review and approve your application.
Can I try out while my clearance is being reviewed?
No. You must be fully cleared with a confirmation email before participating.
I’m transferring—what is the 510 form?
The CIF 510 Transfer Form verifies eligibility after transferring schools. It must be completed and submitted to admin to review before you can be cleared. See instructions above.
