CAHS Athletic Clearance

No student-athlete will be allowed to participate in any sport tryout, practice, or event until they have completed the Athletic Clearance Process AND received an email confirmation! 

Acquiring a CLEARED status requires completing the online portion in HomeCampus and obtaining a pre-participation physical evaluation (PPE) performed by an approved healthcare provider licensed in the state of California (MD, DO, NP, or PA only). 

IMPORTANT: A printed copy of your Athletic Clearance Form needs to be taken to your doctor appointment. Once completed & signed, you will upload the Athletic Clearance Form to your HomeCampus account. 

All athletic clearance paperwork must be uploaded digitally to HomeCampus – no exceptions!

Parents of new/incoming Caiman student-athletes will create a HomeCampus account, input the required information, and upload the necessary forms (see directions below).

Parents of returning Caiman student-athletes will update their previous HomeCampus account and upload a current Athletic Clearance Form (see directions below). 

Each student-athlete requires their own HomeCampus account & is responsible for keeping an updated profile. 

It is the responsibility of the parent/guardian to update/upload the necessary forms to HomeCampus in order to maintain athletic eligibility.

Athletic Clearances expire at the end of every school year. 

Medical Eligibility expires 365 days from your previous physical (PPE).  The Athletic Clearance Form you upload must be dated to last through the entire season you are participating in. If the form expires during the season of participation, the student-athlete will be ineligible for athletic participation until an updated Athletic Clearance Form is uploaded to HomeCampus.

Make sure you have your application for athletic clearance submitted a month before the first day of tryouts for your sport. Please get this completed as soon as you can – Earlier is better!

Step-By-Step Directions

New/Incoming Student-Athletes:

  • Go to https://www.homecampus.com/login and click “Create an Account
  • Both the student-athlete AND the parent/guardian must create HomeCampus accounts.  
    • Input a valid email address, and create your username and password. 
    • You will be asked to verify your email address during login. 
  • Once your account is created and you have successfully logged in, click on “Start Clearance Here
  • Select the school, “Classical Academy High School”, confirm both the school address and academic year. 
  • Select the desired sport. If registering for multiple sports, click “Add New Sport”. 
  • Input the appropriate information for the student-athlete, including insurance information, emergency contacts, and medical information. Make sure to click “Save and Continue” between all steps. 
  • Both the student-athlete and the parent/guardian are required to sign and acknowledge multiple documents on the “Signatures” section.

  • Download and print the Athletic Clearance Form. Bring the form to your appointment! (Some sections need to be completed by the student-athlete and parent/guardian, others can only be completed by the healthcare provider during the physical examination). All student-athletes must have a Pre-Participation Physical Evaluation (PPE) performed by an approved healthcare provider (MD, DO, NP, or PA) licensed in the state of California. 
  • Once completed, upload the completed Athletic Clearance Form as a .pdf under the “Files” section. 
  • You will receive a confirmation message that your clearance is READY FOR REVIEW, however THIS DOES NOT MEAN THE STUDENT-ATHLETE IS CLEARED FOR SPORT PARTICIPATION. The application must still be reviewed by school administration. Once approved, a confirmation notification will be sent.

Returning Student-Athletes:

  • Go to https://www.homecampus.com/login and login to your existing HomeCampus account.
  • Both the student-athlete AND the parent/guardian must update their information on HomeCampus.  
  • Once you have successfully logged in, click on “Start Clearance Here
  • Select the school, “Classical Academy High School”, confirm both the school address and academic year. 
  • Select the desired sport. If registering for multiple sports, click “Add New Sport”. 
  • Make sure all information for the student-athlete, including insurance information, emergency contacts, and medical information is up-to-date. Make sure to click “Save and Continue” between all steps. 
  • Both the student-athlete and the parent/guardian are required to sign and acknowledge multiple documents on the “Signatures” section.
